Life Insurance Claim Checklist
   
After selecting a life insurance policy, review it with your immediate family members.
   

Keep the following information and this checklist in a safe place that your immediate family knows about and are able to access:
     
  Your life insurance and annuity policies
     
  Your insurance companies contact information
     
  Your will and any financial instructions
     
  Your doctors’ names and contact information
     

After your death a family member should request roughly 5-10 copies of your death certificate for your bank, the Social Security Administration, Cross and your insurance carrier’s records.
   
Contact Cross as soon as possible.
